HEALTH-10853 - USA (Maine) - Acquired Brain Injuries and their Families Support Persons Services - Deadline October 30,2026

(1) Vendor needs to provide acquired brain injuries and their families support persons services to the government authority located in Maine.– Services and supports designed to assist individuals with an acquired brain injury (ABI) and their families in connecting with the support needed to live meaningful, productive lives in the community.– Provide Statewide information and referral services through a telephone hotline, that at minimum operate between the hours of 8:00 a.m. and 4:45 p.m. ET.– Provide community resource coordination (outreach and information) to hospitalized acquired brain injury (ABI) survivors including written and electronic material describing available post–hospitalization supports and services with applicable contact information.– Provide Neuro–Resource Facilitation to aid individuals and families with identifying and connecting them with appropriate resources within the community.– Conduct educational and informational programming and discharge planning clinics in participating acute and rehabilitation hospitals.– Provide community outreach and training through facilitation of monthly statewide brain injury training for area agencies and provider staff with focus on basic brain injury knowledge and focused specialty areas such as substance abuse and challenging behavior.– Coordinating electronic and written material that can be made available for support group attendees on an ongoing basis.(2) All questions must be submitted no later than October 1, 2026.(3) A pre–bid informational meeting will be held on September 30, 2026.(4) The contract period will be for one year.
