(1) Vendor needs to provide care management and value–based care programs services to the government authority located in Illinois.– Provides a wide range of health care services– Cultivate innovative care management entities, integrate with providers and embed care management within the medical home and clinical treatment team wherever possible– Increase post–discharge follow–up visits and other post–acute service utilization– Decrease avoidable readmissions through prompt and effective provider/health plan collaboration– Improve member self–management and/or enrollment in a complex care management program(3) All questions must be submitted no later than August 19, 2026.
