Vendor needs to provide remote patient monitoring nursing service to the government authority located in Fairbanks, AK. – For a remote patient monitoring (RPM) nursing service who will provide agency patients with uncontrolled hypertension (approximately 160 patients) the following: cellular enabled blood pressure cuffs, blood pressure monitoring linked to agency Electronic Health Record (EHR), Athena, and individualized health promotion coaching intervention.– Provide and integrate remote patient monitoring software into Athena. – Assistance with enrolling agency uncontrolled hypertension patients into its remote patient monitoring software through December 31, 2026, to include mailing of cellular–enabled blood pressure cuffs directly to enrolled patients and providing assistance to the patient to ensure cuff is linked to the patient's EHR. – Initiate monitoring and individualized health coaching of hypertensive patients enrolled in remote patient monitoring through June 30, 2027.
