Vendor needs to provide street outreach service to the government authority located in New York. – These services must utilize a community–based approach aimed at reducing the harm associated with drug use, particularly in public spaces.• Engaging individuals in active use in the community at physical locations, i.e., at local parks, on the street, and identified hot spots.• Participating in HRMT (High Risk Mitigation Team) meetings and collaborating with the team on street outreach coordination efforts. This includes accepting referrals from HRMT and engaging individuals in active use in harm reduction and treatment services.• Providing harm reduction education to community members.• Facilitating syringe exchange access.• Linking to medication for opioid use disorder services.• Distributing naloxone and maintaining naloxone boxes in the community.• Providing case management services as defined below.• Providing transportation to intake/admission appointments as needed.
