(1) Vendor needs to provide transitional housing program services for transitional aged youth to the government authority located in Ukiah, CA.– Provide Transitional Housing Program (THP) Services to young adults who are homeless or at risk of homelessness.– The program is designed to ensure young adults have a safe living environment to further develop life skills related to independence, prevent homelessness, achieve stability, and enhance long–term success for eligible participants.– Outline screening procedures, including background checks, home assessments, safety evaluations, and ongoing monitoring – Systems to secure housing for young adults as quickly as possible after enrolment, including landlord negotiation, unit identification, and housing readiness coaching. – Provide ongoing monitoring of placement safety, participant well–being, lease compliance, and service effectiveness. (2) All the questions must be submitted no later than September 14, 2026.
