To ensure a fair and open bidding process and that all vendors receive identical information, all questions and bids regarding this Form 470 must be submitted by email to rnakamura@paideiamail.com ; telephone inquiries will not be accepted. Bids are due by 5:00 p.m. MST on the Allowable Contract Date (ACD). All vendors must complete and submit the E-Rate Service Provider Bid Response Worksheet with their proposal. There will be no bidders conference, job walk, or product/service demonstrations, either in person or remotely. Vendors must comply with all requirements outlined in the attached RFP, and failure to do so may result in disqualification. Quantities listed on the Form 470 are estimates and do not guarantee a purchase or award. Bids from vendors that are debarred, suspended, or otherwise prohibited from public procurement may be rejected. All proposed equipment and services must comply with applicable FCC E-Rate national security and supply chain requirements under 47 CFR 54.9 and 54.10. Bids containing equipment or services prohibited by the FCC, including equipment from entities identified on the FCC Covered List, or equipment manufactured for use outside the United States, will be disqualified. Vendors are responsible for ensuring that all proposed equipment and services meet these requirements.
