– Vendor needs to provide adult and youth homeless outreach team’s service. – Provide coordinated housing–focused outreach services that provide direct field engagement, safety, stabilization, and connection to housing and homeless, behavioral health, public health, and social service supports.– Engage and connect individuals living with behavioral health conditions to mental health and substance use services.– Provide countywide, youth–centered outreach and navigation that improves access to coordinated entry, housing resources, and supportive services.– Conduct consistent field–based engagement.
