(1) Vendor needs to provide physical therapist services to the government authority located in Belcourt, ND.– Provide a free appropriate public education to students who have identified disabilities and by reason thereof require specialized instruction including related services to meet their unique educational needs.– Services will include direct therapy to students, consultations with staff and parents, evaluations and assessments, providing documentation and service logs of therapy sessions and offer compensatory time.– Observes, screens, evaluates students and provides documentation of referrals.– Design and implement therapy plans that include a comprehensive narrative detailing the student's specific needs, clearly defined and measurable goals, as well as accurate present level of performance statements.– Provide training and ongoing support to both staff and parents.(2) The contract period will be for one year.
