(1) Vendor needs to provide HIV epidemic program services to the government authority located in OH. • Resource navigation services: Resource Navigation ensures individuals with increased vulnerability to HIV acquisition receive interventions and education that disrupt the medical and social conditions that allow HIV transmission to occur.• Peer navigation/health education and risk reduction services: Provide education to clients living with HIV about HIV transmission and how to reduce the risk of HIV transmission. It includes sharing information about medical and psychosocial support services and counseling with clients to improve their health status.• Health insurance navigation services: Provide guidance and assistance in connecting clients to the most appropriate medical insurance that best meets their needs.• Early intervention services: Provide guidance and assistance to ensure that persons more vulnerable to HIV acquisition know their HIV status and are provided referral and linkage to HIV care and treatment services if found to be living with HIV.• Emergency financial assistance services: Provide Emergency Financial Assistance provides limited one time or short–term payments to assist client with an urgent need for essential items or services necessary to improve health outcomes.• Medical transportation services: Provide nonemergency transportation services that enables an eligible client to access or be retained in core medical and support services.• Housing services: Housing provides transitional, short–term, or emergency housing assistance including utility assistance to enable a client or family to gain or maintain outpatient/ambulatory health services and treatment, including temporary assistance necessary to prevent experiencing homelessness and to gain or maintain access to medical care.(2) All the questions must be submitted no later than October 19, 2026.
