(1) Vendor needs to provide on–site and near–site health clinic services to the government authority located in Midland, TX.– Provide an On–Site/Near–Site Health Clinic (the “Clinic”) and related services for employees.– On–site or near–site health clinic as an enhancement to the district’s comprehensive employee benefits program.- Reducing employee absenteeism and supporting improved employee health, engagement, and productivity; - Increasing access to convenient, high–quality primary care and preventive health services for eligible employees and health plan members; and - Increasing the District’s ability to manage and control primary care utilization and costs, with the potential to improve overall health plan value and reduce long–term health care expenditures. - Physician/nurse “reach out” program to reach members with highest risks. - Disease/case management. - Annual and ongoing preventive screenings. - Provider health seminars. - Provide medical care to members for acute conditions. - Provide medical care to members with chronic conditions. - Specialty Services. - Provide Mental Health Services. - Offer virtual visits as an option for Mental Health Services. - Conduct pre–employment physicals as requested.
