HEALTH-10766 - USA (Chicago, Illinois) - Behavioral Health Services - Deadline October 2,2026

(1) Vendor needs to provide behavioral health services to the government authority located in Chicago, IL. – Provide integrated behavioral health care coordination, resident engagement, crisis response, and supportive services to residents.– The services shall be designed to promote resident stability, address identified behavioral health needs, increase access to appropriate services, and strengthen connections between residents and available health and community resources.• Rapid Response and Crisis Stabilization– Provide rapid response services within two business days for residents experiencing pre–crisis conditions or behaviors that may be escalating to unstable or unsafe situations.• Behavioral Health Care Coordination and Resident Support– Provide ongoing care coordination and supportive services to participating residents based on their individual needs.• Resident Outreach and Engagement– The Contractor shall conduct proactive, in–person outreach to residents who may benefit from behavioral health or supportive services but have not engaged with available resources.• Consultation and Education for agency Staff– provide consultation and education to agency property management staff and social service providers regarding behavioral health concerns and systems of health care.• Primary Care Referrals– Provide referrals and linkages to general primary care services to support residents' preventive health needs and address acute and chronic health conditions.(2) A pre–bid conference will be held on September 9, 2026.(3) All questions must be submitted no later than September 11, 2026.(4) The contract period will be three years.
