(1) Vendor needs to provide levels of care assessment tool implementation services.1. Hospital identification and engagement:– Identify eligible birthing hospitals, secure their participation, and establish facility–level implementation leadership through designated perinatal nurse champions.2. Self–assessment administration and data collection:– Coordinate completion of the self–assessment instrument at all participating hospitals and validate data for completeness and accuracy.3. Data analysis and gap identification:– Collaborate to analyse findings, characterize the current landscape of maternal and neonatal care capacity in region and identify gaps, disparities, and opportunities for improvement.4. Report development and dissemination: – Produce a comprehensive written assessment report that presents findings, identifies priorities, and recommends facility and regional–level improvements aligned with hub action plan goals.5. Data reporting and performance tracking:– Collect and submit all required performance metrics to monthly throughout the contract period.(2) All question must be submitted no later than August 06, 2026.(3) A contract period will be for one year.
