(1) Vendor needs to provide member pathways, advocacy, support, and stability service to the government authority located in Cheyenne, WY.– Coordinating outcomes: member pathways, advocacy, support, and stability previously known as the care management entity will support medicaid–eligible children and youth with serious mental and behavioural health challenges.– Medicaid youth at risk of out–of–home placement.– Medicaid youth who currently meet psychiatric residential treatment facility level of care or are placed.– Medicaid youth who meet acute, psychiatric stabilization hospital level of care; had an acute hospital stay for mental or behavioural health conditions in the last 365 days; or are currently placed in an acute hospital stay for mental or behavioural health conditions.– Youth on the children’s mental health waiver.– Kid care children’s health insurance program.– Children and youth with serious emotional and behavioural health challenges often receive fragmented care of various services, contributing to poor outcomes and unnecessarily high costs.(2) All the questions must be submitted no later than September 15, 2026.
