EXTRA-76805 - USA (Woodbury, New Jersey) - Ophthalmology Service - Deadline December 9,2026

(1) Vendor needs to provide ophthalmology service to the government authority located in Woodbury, NJ. – Provide eye care services to the residents of home in accordance with local, state and federal regulations.– Ophthalmologist will provide a copy of their license and current malpractice insurance.– The ophthalmologist or his/her designee will notify the administration of any changes in services that affect the facilities residents including to any change in eye care.– Have the ability to use the sigma care system by writing orders and imputing documentation into the system.– The ability to bill medicare, medicaid and private insurance.(2) The contract period will be for two years.
