(1) Vendor needs to provide inpatient hospice services to the government authority located in Bellaire, TX.• Many hospice–eligible patients can transition to community–based hospice programs, residential hospice facilities, nursing facilities, or home hospice services.a) Absence of caregiver or family support;b) Lack of a safe discharge location;c) Housing instability or homelessness;d) Financial or insurance limitations;e) Medical complexity requiring ongoing inpatient management;f) Behavioral or placement barriers; org) Limited availability of appropriate hospice placements• Improve Access and Continuity of Care: Expand immediate access to comprehensive hospice care for hospitalized patients who elect comfort services but face social, financial, or placement barriers to external discharge, ensuring continuous, seamless management within facilities.• Patient–Centered–End–Of–Life Care: Support the delivery of high–quality, dignified comfort care that prioritizes intensive symptom management, pain control, and strict alignment with patient and family goals of care.• Interdisciplinary Team Coordination: Enhance clinical communication, care planning, and workflow coordination by integrating specialized hospice physician expertise and hospice resources directly with hospital physicians, nurses, palliative care, case management, and pharmacy teams.• Family and Psychological Support: Strengthen critical support mechanisms available to patients and families within the acute environment, including dedicated social work, counseling, advance care planning, and structured bereavement resources.(2) The contract period will be for one year.
