(1) Vendor needs to provide operation and program administration services to the government authority located in Bunnell, FL. – Operate a community–based adult day care program designed to provide a structured, protective environment for eligible adults • Core operating schedule:– Mandatory operational hours: The facility shall remain open to participants from 9:00 AM to 4:30 PM, Monday through Friday, excluding agency–designated holidays.• Participant intake, assessment, and care planning:– Eligibility and intake screening: Conduct comprehensive pre–admission screenings to ensure applicants meet adult day care criteria.• Clinical, health, and medication management: – Nursing oversight: Provide on–site or contracted nursing coverage (RN or LPN) to perform daily health monitoring, including vital sign checks, blood sugar monitoring, and weight tracking as specified by individual care plans.• Activities of Daily Living (ADLs) and personal care assistance:– Direct care support: Provide respectful, hands–on assistance with basic Activities of Daily Living (ADLs).• Therapeutic, social, and memory care programming:– Daily structured activities: Design and execute a minimum of three hours of scheduled therapeutic activities per day.• Nutritional and dietary services:– Meal requirements: Provide a minimum of one hot, nutritionally balanced meal and two healthy snacks during the designated operational day.• Facility operations, maintenance, and security: – Utility financial responsibility: Pay all facility utility costs – including electricity, water, sewer, internet, and telephone service–directly to providers.• Safe transportation services.• Emergency activation and disaster assistance response:– Hurricane and evacuation shelter support: In the event of local hurricane activations, severe weather emergencies, or mandatory county evacuations ordered by agency, the agency may request the vendor to assist in emergency shelter operations.(2) All the questions must be submitted no later than August 14, 2026. (3) The contract period will be for three years. (4) Mandatory pre–bid meeting will be held on August 12, 2026.
