EXTRA-76071 - USA (Los Angeles, California) - Community Engagement Services for the Uninsured Program Services - Deadline September 16,2026

(1) Vendor needs to provide community engagement services for the uninsured program services to the government authority located in Los Angeles, CA.– Provide comprehensive Primary Health Care Services.– Which includes preventive care, routine and urgent primary care visits, chronic disease management, mild–to–moderate behavioural health services, nutrition education and services, ancillary lab services, basic durable medical equipment and care coordination.– The Contractor shall be responsible for caring for Participants at their assigned Medical Home, maintaining continuity of care, managing overall health, and coordinating access to additional services when needed. Primary Health Care Services shall be provided in–person or if applicable, telehealth (video or telephone).– Nutrition services shall be incorporated into primary and preventive care, particularly for chronic disease management.– Services provided in an outpatient setting to Participants for the prevention, diagnosis, or treatment of illness or injury including health evaluations, health advice, therapeutic services, diagnostic services, routine and preventive services, health care maintenance, chronic disease management, immunizations, outreach, emergency first aid, information and referral services, health education, prescribing medicines and other related services.
