HEALTH-10212 - USA (Cole, Missouri) - Physical Therapy services - Deadline June 15,2026

(1) Vendor needs to provide physical therapy services to the government authority located in Cole, MO.• Provide a Missouri certified teacher of the blind and visually impaired in compliance with agency in good standing throughout the contract period to provide vision services• The contractor and/or therapist shall be responsible for any fees related to the Family Care Safety Registry• The fingerprint check registration and actual fingerprint must be completed.• Therapists provided by the contractor should have experience providing therapy services• Services shall be provided with an integrated approachin conjunction with the teacher in the classroom or most appropriate educational space• The contractor shall coordinate and supervise all aspects of the requirements stated herein in accordance with the agency• Students are available at school Monday–Friday from 9:00 a.m. to 3:00 p.m. In–school services must be provided withinthe school calendar and these timeframes.• Services should not begin before 9:15 a.m. and should end by 2:45 unless thetherapy is specifically related to the students The contractor shall agree and understand that the times and days on whichthe contractor and/or therapists provides services(2) The contract period will be for one year.
