(1) Vendor needs to provide community health assessment service to the government authority located in Belleville, NJ. • Collect, analyze, and report community health data in order to identify health needs, disparities, community strengths, and opportunities for improving the health and well–being of residents.• Develop and customize appropriate data collection tools, including resident surveys, focus groups, and key informant interviews• Identify and utilize relevant primary and secondary data sources.• Develop a data collection and sampling plan.• Data collection tools should be available in English and Spanish.• Conduct quantitative and qualitative analysis of collected data.• Identify social determinants of health and built–environment factors where applicable.• Identify community assets and resources that may assist in addressing identified health challenges.• Resident survey data;• Available local health data;(2) The contract period will be for one year.
