(1) Vendor needs to provide pre–employment physical examinations services to the government authority located in Anchorage, AK.• All physical examinations and evaluations must include:• Patient contact information forms.• HIPPA privacy notices as required under applicable law.• Consent form for examination or evaluation.• Release of information form.• Physical fitness opinion form, signed by the contractor’s medical provider.• The contractor should order those additional screening tests necessary to ensure the applicant's or employee’s health is adequately assessed. Abnormal findings during a physical examination or evaluation must be addressed by follow–up or referral.(2) All question must be submitted no later than July 22, 2026.(3) A contract period will be for one year.
