Description
PLEASE NOTE – this Request for the Application is only for new applicants to apply to hold the Residential Support Services contract. Current Residential Support Services contract arrangements stand. Attached is the Request for Application (RFA) to apply for a Residential Support Services contract with ACC. Residential Support Services (RSSA) provides residential care for Clients who are unable to live independently in the community or are unable to live in their usual place of dwelling due to their covered physical injury including a Traumatic Brain Injury. The RSS Service is comprised of two key components: • Rehabilitation that maximises Client independence and prepares them for living in the community; and • Residential Care for when a Client is unable to live independently in the community. This may include: - Home for Life, - Respite Care, - Temporary Accommodation, (for example, until the Client’s housing modifications are complete or a suitable home is found), - Interim Care, (for example, for Clients who have been discharged from a District Health Board for a period of convalescence before returning to the District Health Board for ACC funded rehabilitation). If you have any questions regarding the RFA documents, please ask these through the GETS Question & Answer function.
