(1) Vendor needs to provide athletics secondary insurance services to the government authority located in Ogden, UT.– Provide Secondary medical insurance for Intercollegiate Athletics injury.– Secondary medical insurance for Intercollegiate Athletics injury.– Dental coverage.– Coverage of prescribed orthopaedic appliances.– Coverage of prescription costs submitted.– Coverage of physical therapy costs submitted.– Coverage for usual, customary, and reasonable expenses assistance surgeon, ambulance, chiropractor, podiatrist, etc.– Administrative costs to process claims including negotiating discounts with doctors and hospitals.(2) The contract period will be for one year.
