(1) Vendor needs to provide in–home health care and support services to the government authority located in Hawaii. – Design and implement a Pilot Program and provide in–home health care and support services for elderly individuals who do not qualify for existing programs, such as Medicaid and cannot afford in–home care services.– The design shall include intake, navigation, and care coordination that includes but is not limited to the following:• Intake Call Center to receive referrals• Process eligibility determinations• Documentation of all interactions• Ability to assess eligibility for elderly individuals who are not eligible for Medicaid, cannot afford private care, and considers early–intervention focus.• Provides warm handoffs and documented referrals for non–eligible individuals to appropriate public programs or community resources• Assign trained staff to conduct in–home assessments, develop individualized POC, stratify tasks among caregivers, and monitor progress(2) All the questions must be submitted no later than August 19, 2026.
