Vendor needs to provide licensed funeral director services to the government authority located in Shawnee, OK.• Director by the state registrar of vital records for the state.• The funeral director shall work with the city to ensure that, before the disinterment process takes place, the next of kin has been properly notified.• The funeral director shall obtain proper forms from the state registrar of vital records before disinterment and reinterment processes take place.• If next of kin has been properly notified and does not elect to move remains to a different cemetery, reinterment within the cemetery shall be proper.• If the remains are to be placed in the cemetery, the funeral director shall ensure a proper notice of disinterment and reinterment is completed, signed by the funeral director and next of kin, and is submitted to the state registrar of vital records at the state department of health within five days of such action.• If next of kin elects to move their deceased kin’s remains to a different location, the Funeral director will work with the family to ensure the proper transfer of remains to the alternate burial site.
