(1) Vendor needs to provide transportation services to community–based crisis programs to the government authority located in Rochester, NY.– Providing transportation services to community–based crisis programs for individuals experiencing a low–acuity substance use disorder (SUD), opioid use disorder (OUD), and mental health (MH) crisis.– Improve the behavioral health and well–being of residents living in County by providing easier access and support getting to specific community–based crisis programs, while intentionally addressing disparities based on race and income.– Seeking a provider to host and maintain crisis transportation, call center to triage calls, and projectmanagement components of this project.– identified call center will accept incoming calls from the FIT and PIC Teams to inquire about potential transport to a community–based crisis program for clients experiencing low–acuity SUD, OUD, and MH crisis.– Call center will maintain a database of endpoint resources, services provided, capacity, and vacancies for those resources, patient eligibility, and when transportation and peer specialist will be available.– Call center will inquire that the appropriate crisis program is available and then will initiate a transport for the selected respondent and peer specialist.– Provide an opportunity for system evaluation and further behavioral health data collection.– Peer Specialist will provide a warm hand–off to the crisis program and a follow–up phone call 24–48 hour later.(2) All questions must be submitted no later than August 7, 2026.(3) The contract period will be for one year.
