(1) Vendor needs to provide senior affordable housing tenant services to the government authority located in New York.1. Program expectations:– The contractor would ensure that the program has an appropriate staffing plan with sufficient numbers of staff to meet an approximate 1:20 assistance ratio of staff to formerly homeless tenants.2. Program evaluation and reporting:– The contractor would track, record, and report information to the city as required, utilizing their own and city–identified systems including, but not limited to, tenant demographics, income source, place of discharged and outcome data, including occupancy rate; housing retention/stability; hospitalization rate.3. Budget management:– The proposed budget represents the annual costs to provide services for the proposed program.(2) A contract period will be for five years.
