(1) Vendor needs to provide chronic disease self–management program coordination services to the government authority located in Hilo, HI.– The applicant shall provide the necessary qualified staff, equipment, vehicles and facilities that will be required in carrying out this service.– All personnel must be trained, screened and deemed competent by the applicant to provide the services as described in their job descriptions.(2) All the questions must be submitted no later than August 13, 2026.(3) The contract period will be for one year.
