(1) Vendor needs to provide diabetes self–management education and support services to the government authority located in Houston, TX.– Deliver Diabetes Self–Management Education & Support programming using approved curriculum– Become or maintain an accredited Diabetes Self–Management Education & Support site.– Recruit and enroll participants from priority populations– Conduct at least five Diabetes Self–Management Education & Support cohorts– Track attendance, engagement, and outcomes– Ensure compliance with HIPAA/data privacy requirements– Coordinate with HHD and clinical partners
