(1) Vendor needs to provide wound care physician services to the government authority located in West Lafayette, IN.– Provide physicians in the specialty area of wound care to the facilities’ residents– Services will include but are not necessarily limited to:a. Perform patient consultations and follow–up visits.b. Take part in Quality Assurance programs and other sta􀆯 programs, as reasonably requested.(2) The contract period will be for two years.
