(1) Vendor needs to provide licensed clinical social worker services to the government authority located in Lyons, NY. – The facility staffing includes social workers who are experienced in the long–term healthcare setting but requires a licensed clinical social worker to provide independent counseling to residents, families, and staff, and assist with resident diagnosis, while also providing a variety of educational activities that focus on facility culture.– Provide licensed clinical social worker (LCSW) services, including psychosocial support, education, and independent counseling services to residents, families, and staff.– Provide consultation and supportive therapy to residents referred within the facility– Attend staff meetings with social work and activities– Home and work issues that impact provision of care.– Facilitate staff education per administration– Mental health history or new diagnosis.– Transition difficulties.– Family dynamics.– Resident–to–resident concerns.– Concerns with staff. – Provide support to staff.(2) All the questions must be submitted no later than September 10, 2026.(3) The contract period will be for three years.
