(1) Vendor needs to provide employee assistance program service to the government authority located in Phoenix, AZ. – Provide a comprehensive employee assistance program.– Provide a designated account manager and account management team.– Provide a website for use by employees and eligible dependents to obtain information on the program including behavioral health and wellness information.– Provide communication strategies and materials that include at a minimum employee brochures and supervisor manuals.– Provide exceptional customer service support via phone, email and website.– Provide issue–specific face–to–face counseling services to employees and eligible dependents through professional counsellors.– Provide a minimum of six one–hour face–to–face visits per issue, per employee or eligible household member each calendar year.– Provide 24–hour per day, 365–day per year toll–free telephone crisis counseling.– Provide referrals to qualified, behavioral health providers in the district's medical plan networkupon request of the employee or eligible dependent requiring continuing care following the initial six visits.(2) All the questions must be submitted no later than December 3, 2026.
