(1) Vendor needs to provide annual physical exams services to the government authority located in Kauai, HI. – An estimated quantity of 145 physical examinations shall be conducted annually. • Physical exam:– Physical examination– Medical history review– Skin cancer assessment– Health promotion counseling– Sleep disturbance apnea questionnaire– Cardiac risk stratification– OSHA resp. questionnaire / clearance letter– Firefighting medical clearance• Ultrasound screening:– Thyroid– Carotid Artery– Aortic Aneurysm Appearance– Kidney• Laboratory blood / urine testing:– Complete Blood Count w/ Differential– Comprehensive Metabolic Panel– Fasting Lipid Panel (LDL, HDL, Trig, Cholesterol)• Occupational health testing:– Audiogram (OSHA Rated Hearing Booth)– Biometric Screening (BMI or Calipers Skinfold)– Spirometry (Pulmonary Function Test)• Cardiopulmonary / fitness evaluation: Stress EKG, WFI Protocol.(2) All the questions must be submitted no later than August 20, 2026. (3) The contract period will be for three years.
