(1) Vendor needs to provide rural health transformation program rural partners services to the government authority located in Boston, MA.– Strengthen access to healthcare, improve population health, and advance long–term health and well–being across rural state.– Ensuring rural residents can access needed healthcare services, generating opportunities to improve the health and well–being of rural residents and scaling systems, policies, and investments to meet the unique needs of rural communities.– Serve as the primary point of coordination for onboarding, workplan development, funding administration, technical assistance scheduling, reporting expectations, and ongoing implementation support.– Fiscal monitoring.– Support to strengthen fiscal management practices, establish appropriate tracking and documentation processes, understand reporting requirements, and address identified gaps.– Performance monitoring may include periodic progress reports, participation in monitoring or technical assistance meetings, submission of quantitative and qualitative data, documentation of individuals or communities served, progress toward identified milestones and outcomes, identification of implementation barriers, and supporting documentation demonstrating completion of key activities.(2) The contract period will be for five years.
