(1) Vendor needs to provide housing–focused outreach services that provide direct field engagement, safety, stabilization, and connection to housing and homeless, behavioural health, public health, and social service supports.– Housing–focused Street outreach is an engagement strategy that prioritizes connecting people experiencing unsheltered homelessness to lifesaving, person–centered, and culturally responsive housing, mental health and substance use, public health, and social services and resources while actively working toward stable and permanent housing solutions.– The Housing Focused Outreach Teams will also be responsible for conducting targeted, on–going field–based outreach that engages individuals living with substance use disorders and facilitates rapid access to low–barrier substance use treatment, including Medications for Addiction Treatment• Reaching and serving high–need priority populations• Increasing access to substance use disorder services, housing interventions, and evidence–based and community–defined practices, and building the behavioral health workforce• Focusing on outcomes, transparency, accountability, and equity– Conduct consistent field–based engagement (encampments, street locations, shelters/drop–in points as appropriate) using nonjudgmental, autonomy–centered practices– Connect people experiencing homelessness to the and the system of care; provide CE–approved assessments and prioritize revisions/decliner forms when appropriate.
