(1) Vendor needs to provide pre–employment and annual physical examinations service to the government authority located in Arvada, CO. – Provided include to pre–offer and fit–for–duty physical examinations for firefighters, drug and alcohol screening, urban search and rescue physicals and hazardous materials employee examinations.– Provider’s medical employees and staff must be professionally trained and hold valid licenses to practice.– Physical examination process is designed to examine candidates and employees for current and continuous employment to determine their ability to perform the job duties according to the specific position.– Conduct medical examinations according to accepted industry and medical standards and according to the physical examination protocols determined.– This would also include annual exams for safety employees.– Physical examinations must be scheduled within five (5) business days of being requested. – Occupational health consultation services. – Medical services.– Medical health records.(2) All the questions must be submitted no later than September 04, 2026.
