HEALTH-11036 - USA (Gilpin, Colorado) - Residential Treatment and Targeted Case Management Service - Deadline November 9,2026

(1) Vendor needs to provide residential treatment and targeted case management service to the government authority located in Gilpin, CO.• Provide residential treatment and targeted case management for co–occurring substance use disorder (SUD) and mental health needs for individuals on probation• The program should work to stabilize the Probationer, provide treatment and supervision plans, assist with educational or employment preparedness and discuss discharge plans with the Probationer.• The goal is to provide the Probationer with preliminary stability of mental health and substance use needs to assist with on–going probation supervision.• Residential Treatment: Contractor shall provide residential co–occurring substance use and mental health treatment servicesi. Contractor shall provide residential housing for up to ten (10) Probationers ata time, in a group setting with supervision, for up to ninety (90) days perProbationer unless a longer period of time is approved in advanceii. Contractor shall provide three (3) meals per day per Probationer and accessto bathroom, shower, and laundry facilities.iii. Contractor shall follow evidence–based practices and treatment modalities.iv. Contractor shall offer a variety of evidence–based treatment modalities (withan emphasis on Cognitive Behavioral Therapies).v. Contractor shall complete a clinical evaluation and assign treatmentservices within seven (7) business days of the client’s program intake.vi. Contractor shall demonstrate integration of a family/support systemcomponent of treatment.vii. Contractor shall identify mental health needs that cannot be addressed byContractor and shall make referrals to mental health/psychiatric services asinstructed by Department.(2) The contract period will be for one year.(3) All the questions must be submitted no later than October 28, 2026.
