(1) Vendor needs to provide brain health program evaluation services to the government authority located in Bloomington, MN.• Provides services through licensed clinicians, supervised graduate clinical therapy interns, crisis follow–up, care coordination, warm handoffs, and community partnerships. • Services typically are for 12 weeks and include 10–15 therapy sessions, referrals and resources for continued care, and group therapy or psychoeducation for high school students. • The evaluation will assess program structure and participant outcomes from enrolment through program completion and at sustained follow–up intervals. (2) Vendor have to registered on issuing authority website in order to download unwatermarked RFP.(3) A contract period will be for two years.
