(1) Vendor needs to provide discharge planning services. to the government authority located in San Francisco, CA.– Comprehensive jail discharge planning, diligent case management, and strong partnerships with community–based programs are critical to connecting vulnerable reentry populations to relevant, effective services. Incorporating housing support as a component of the spectrum of services can often be the determining factor between success and recidivism.• Clients respond well to culturally competent, non–judgmental services, delivered in a manner that demonstrates respect for individual dignity, personal strength, and selfdetermination.• Because those engaged in unsafe health practices are often difficult to reach through traditional service venues, reentry services providers will seek creative opportunities and develop new strategies to engage, motivate and intervene with potential clients.• Relapses or periods of return to unsafe health practices are not equated with or conceptualized as “failure of treatment”.– Data ManagementThe successful proposer shall maintain case notes for every client interaction in its information management system to capture process outcomes such as the number of persons declining assistance and how many times they were approached.• Provide referrals and linkages to in–custody and community–based organizations;• Convene case conferences with in–custody and community–based providers;• Collaborate effectively with the medical and legal system and with community providers;• Communicate with staff from the Department of Homelessness and Supportive Housing’s Jail Access Center for Coordinated Entry.(2) A mandatory virtual conference will be held on September 14, 2026.(3) All question must be submitted no later than September 18, 2026.
