(1) Vendor needs to provide local adult re–entry coordination and services to the government authority located in Rochester, NY.• A Local re–entry coordinator and case managers to provide re–entry services to 325 clients peryear, including providing these services.• Services to clients may include to: creating a unique service plan; applying for Safety Net, Medicaid and food stamp benefits at least one month prior to Re–entry into the community; assisting in obtaining employment, housing, transportation, clothing, prescriptions, and other personal items; providing appointment reminders; obtaining chemical dependency and mental health evaluations; obtaining vocational education or domestic violence programming; meeting with clients regularly to ensure appropriateness of placements, troubleshooting issues as they arise; and, meeting with clients’ families as necessary.• Engage in strategic planning to develop effective approaches to serving individuals, building capacity and educating the public• Engage the community in building a comprehensive re–entry system through community education and outreach efforts, and to provide a summary of the event(2) All questions must be submitted no later than September 4, 2026.
