EXTRA-72294 - USA (Albany, New York) - Environmental Modifications Evaluators Service - Deadline August 18,2026

(1) Vendor needs to provide environmental modifications evaluators service to the government authority located in Albany, NY.• Agency is seeking competitive proposals from the following types of Offerors, including but not limited to: Independent Living Specialists, Independent Building Contractors, Occupational Therapists, Physical Therapists, Remodeling Contractors and Builders, Engineers, Rehabilitation Specialists, Case Managers, Public Health Nurses, Assistive Technology Specialists, Rehabilitation Evaluation Agencies, and Rehabilitative Specialists Servicesa. Initial Evaluation – An Evaluator must review documentation provided by the Fund Administrator explaining the needs of the enrollee and must make an in–person visit to examine the home in its current state. The Offeror must determine whether the home needs modification in accordance with the regulations governing the MIF.b. Bid Evaluation – The Offeror must review all acceptable General Contractor bids provided (typically three), as defined by regulation, that have been submitted for the construction or installation of an agency(s) as approved by the Fund Administrator. This must include evaluation of the qualifications of the General Contractors.c. Mid–Modification Evaluation – At the discretion of the Department or Fund Administrator an Evaluator must make an evaluation of the construction progress, including any requested change orders. Evaluators must make an in–person visit during this evaluation if required.d. Post–Modification Evaluation – An Evaluator must make an in–person site visit to examine the completed agency to determine whether the agency as installed or constructed meets the enrollee’s functional needs, and follows the initial evaluation, including adherence to safety and all applicable building code standards. An Evaluator must observe and assess the enrollee or representative test the agency and confirm that all approved agency are functional and complete.(2) The contract period will be for one year.(3) All the questions must be submitted no later than July 14, 2026
