(1) Vendor needs to provide physical therapy services, evaluations, director therapy, consultation, and support for students with impairments to ensure equitable access to the educational curriculum to the government authority located in Elizabeth, WV.– Provide in person direct services as per student’s Individual Education Program (IEP).– Conduct initial and triennial assessments.– Collaborate with teacher to complete program required assessments, general education teachers, parents, and administration.– Participate in meetings and ensure all services adhere to federal regulations.– Serve an estimated 20 students across 3 school sites.(2) Contact term will be for one year.
