HEALTH-11138 - USA (Statesville, North Carolina) - Physical Therapy Services - Deadline November 30,2026

(1) Vendor needs to provide physical therapy services to the government authority located in Statesville, NC.– On–site physical therapy services for county employees, retirees, and eligible dependents covered under the county's group health insurance plan.- A low–cost, convenient access point for employees, retirees, and dependents to receive on–site physical therapy evaluation and treatment, including orthopedic rehabilitation, post–operative therapy, chronic pain and injury management, manual therapy, and therapeutic exercise.- Initial evaluations and periodic re–evaluations utilizing appropriate assessment instruments and techniques to establish an individualized plan of care for each patient.- Direct treatment services delivered according to the patient's plan of care, including progress documentation and communication of outcomes as appropriate and permitted by law- An itemized invoice detailing services rendered will be furnished to the County monthly. Services rendered under this agreement will not be billed to the County's health plan.- Services will be available to all active benefit–eligible employees, retirees, and dependents who are covered by the County's health insurance plan.– Current On–Site Physical Therapy Services (For Vendor Reference):- On–site physical therapy evaluation and treatment- Orthopedic rehabilitation- Post–operative therapy- Chronic pain management- Injury prevention and recovery support- Coordination of referrals with the Wellness Clinic and outside specialists(2) All question must be submitted no later than November 13, 2026.
