(1) Vendor needs to provide cloud based post–acute care referral system. • Agency seeking a state–of–the–art automated referral application system to increase productivity.• System must electronically reserve placement of patients, eliminating the need for phone calls, outside emails, and faxes.• System must allow providers to electronically accept referrals without subscription or fees.• System must provide automated responses for receipt and acceptance of referrals from providers• System shall include the ability to manage ambulance/transportation referrals similar to post–acute provider process.• System must provide appropriate security for the transfer of HIPPA data and must meet all HIPPA and data security requirements.(2) All the questions must be submitted no later than Oct 8, 2026. (3) The contract period will be for two years. (4) A Virtual Pre–Submission Conference will be held on September 29, 2026
