(1) Vendor needs to provide provider–facing population health, care coordination, and interoperability solutions. – Collect information about solution options and functionality of population health data systems for use by rural healthcare providers.– Respondents shall provide information regarding capabilities to:1. Receive and integrate Medicaid claims, encounter, clinical, pharmacy, laboratory, immunization, social care, and administrative data.2. Normalize, reconcile, and exchange data received from multiple sources.3. Describe how data is stored, transmitted, processed, accessed.4. Connect with rural hospitals, clinics, specialty providers, behavioral health practices, pharmacies, skilled nursing facilities, and community–based organizations.5. Connect with providers and organizations using different electronic medical record systems.6. Support bidirectional exchange of clinical, care management, referral, and administrative information needed for provider–facing care coordination.7. Exchange information among providers, payers, community–based organizations, and other authorized participants without duplicative interfaces or repositories8. Migrate data into electronic medical record systems or other related systems, when required.9. Maintain, monitor, and support data interfaces after implementation.– Analytics, Quality Measurement, and Payer Functions– Care Coordination, Referral Management, and Community Connectivity– Patient Engagement, Outreach, and Virtual Care(2) All the questions must be submitted no later than July 29, 2026.
