(1) Vendor needs to provide companion care services to the government authority located in Salt Lake, UT.• Companion caregiver services for post–general anesthesia patients.• Involve personal care, transportation and companionship in the absence of the patient’s own caregiver or companion.• Personal caregiving, transportation, and accompaniment services for post–general anesthesia procedure patients.• Include transportation from hospital to home, accompaniment and personal caregiving for four (4) hours post–procedure following hospital discharge.• Hospital may qualify some patients for financial assistance careful screening.(2) All questions must be submitted no later than September 9, 2026
