Vendor needs to provide behavioral health housing units coordinator services to the government authority located in Redwood, CS.• The county provides for the health and welfare of all people within its borders and serves as the local government for the unincorporated areas.1. Tenant transition services:– Assist behavioral health unit applicants with initial interviews and orientation process.– Coordinate across site specific logistics for move–in assistance services.– Other applicable tenant transition services, as necessary.2. Tenant sustaining services:– Tenancy–focused outreach and assessment.– Coordination with a landlord, property manager, and case–management provider concerning housing–stability issues.– Tenant and landlord education; landlord–tenant relationship coaching; and dispute resolution.– Lease–compliance support and eviction issue identification, prevention and intervention (e.g. health/safety visits).– Household–management, cleaning, independent–living, and other life–skills assistance when delivered to maintain tenancy.– Housing–support–plan and housing–related crisis–plan updates.
