CASE-1787 - USA (Nashville, Tennessee) - Case Management Service - Deadline September 9,2026

(1) Vendor needs to provide case management service to the government authority located in Nashville, TN. – Provide 238 Case Management (CM)s across the state in locations determined by agency.– Conduct quality Caseworker/Child visits to include comprehensive assessments with each youth/child and their family to determine their strengths and needs and of the social, financial, and institutional resources available to the client, as well as safety. Frequency and visit requirements are in accordance with agency policy.– Develop and advocate for permanency plans that are constructed in accordance with agency policy.– Appear in court to provide updates and meet state and federal statute for permanency planning and reviews.(2) All the questions must be submitted no later than August 21, 2026.(3) All the Notice of Intent to Respond must be submitted no later than August 18, 2026.
