(1) Vendor needs to provide home health services for the aging and disability to the government authority located in California. – Providers to deliver home health and/or in–home support services for conservatees and other vulnerable adults who require assistance to remain safe, stable, and supported in the community. – Services may include personal care, home–based assistance, and related supports coordinated through the Public Guardian, Adult Protective Services, or other programs within Aging and Disability Services.– Provide non–medical in–home supportive services, as authorized by the County and consistent with the client’s service plan.• Personal Care Assistance • Homemaker and Chore Services • Shopping and Essential Errands • Medication Reminders and Observation • Escort and Accompaniment• Safety Monitoring and Supportive Supervision • Home Organization and Environmental Support • Communication and Observation Support • Other Authorized Supportive Services (2) A non–mandatory pre–bid meeting will be held on November 20, 2026.(3) All questions must be submitted no later than November 25, 2026.
