(1) Vendor needs to provide homelessness and substance use services to the government authority located in California. • The selected proposer will provide outreach, engagement, harm reduction, case management, treatment linkage, and recovery support services to individuals experiencing homelessness and substance use disorders.• Outreach and Engagement– Conduct outreach– Engage individuals experiencing homelessness and substance use disorders– Build rapport and facilitate service participation– Coordinate with agency staff and partner agencies• Substance Abuse Recovery Services– Conduct assessments and screenings– Provide case management and recovery navigation– Connect clients to detoxification, treatment, and behavioral health services• Harm Reduction and Overdose Prevention– Provide overdose prevention education– Educate clients regarding fentanyl risks and overdose response• Housing and Stabilization Support– Connect participants to shelter and housing resources– Assist with obtaining identification, benefits, and supportive services– Coordinate with regional homeless service providers• Collaboration and Reporting– Participate in coordination meetings with agency staff and community partners– Maintain accurate service records and outcome data– Submit required monthly and quarterly reports(2) All the questions must be submitted no later than August 26, 2026.
