(1) Vendor needs to provide better choices better health chronic disease self–management program coordination services to the government authority located in Hilo, HI.– Evidence–based chronic disease self–management program developed and tested by university designed to complement and enhance medical treatment and disease management.– Able to manage their symptoms and have an easier time communicating with their health care providers and loved ones about their condition.– Provide its capacity to keep financial and program records specific to this contract.– Monitored through on–site and desktop assessments to evaluate program and fiscal performance, progress toward meeting goals and to oversee compliance with government requirements and contractual agreements.– Person–centered care.– Coordination of services.(2) All questions must be submitted no later than August 13, 2026. (3) The contract period will be for two years.
